Final Forms registration for the 2024-25 school year is now open to Collierville Schools families who will have a student-athlete participating in district-sponsored athletic programs.
To register for athletics for next school year, login via the link below and follow all directions.
All forms must be completed and an up-to-date physical dated on or after April 15, 2024 must be on file. In addition, students must be on a FinalForms roster for that sport to tryout and participate.
We encourage you to complete this process as soon as you can so that our coaches have the most accurate rosters and emergency contacts heading into the 2024-25 school year.
