FinalForms, the district’s online athletic forms and data management service, is ready for 2025-26.
The platform allows our families and coaches to complete and sign the necessary required athletic forms electronically. Data is saved from last year and pre-populated wherever possible, saving time for next year’s registration.
Users may review data at any time to verify accuracy. All users will be required to sign forms once per year after the open date. A reminder that all forms and physical examinations completed on or after April 15 will be good for the duration of the following school year.
The Parent Playbook is attached. If any support is required during the process, users may scroll to the page bottom-right and click “Get Help”. Please contact your ?coach if additional help is needed.
FinalForms is a requirement for all student-athletes in Collierville Schools prior to participation during the 2025-26 school year.
