FinalForms is the official registration program of Collierville Schools Athletics for online forms , and data management services. FinalForms allows you to register, complete and sign athletic participation forms and review TSSAA and NCAA eligibility forms.
FinalForms most valuable feature saves data from season to season and year to year, meaning you will never need to enter the same information twice. As a time-saver, FinalForms also pre-populates information wherever possible to each of your students. DON'T FORGET, FinalForms allows parents and guardians to digitally upload the medical physical form right into the system without having to drop off the form.
After initial registration, you may review your data at any time to ensure information is current.
At the start of every school year, we require student-athletes and parents to REGISTER their forms once per year and update all the required information. This also allows our coaches and athletic department to contact everyone via email contact lists built into FinalForms.

SPORTS PHYSICALS REQUIREMENT FOR 2023-24
A sports physical is required for all students who participate in sports. A physical examination must be complete on or after April 15, 2023 to be considered a valid physical for 2023-24.
